 How do I make, change or cancel an appointment?

  • Please call (707) 894-4229, 8:00 AM-7:00 PM, to schedule, change or cancel an appointment. If calling after office hours, an after hour message line is available to notify the office of changes needed.
  • If you need to change your appointment, please call the office 24 hours beforehand (or sooner) to reschedule or let us know if you will be unable to keep your appointment.

Did you know that Alexander Valley Healthcare has an online patient portal?

Through the patient portal, you can review laboratory results, make appointments, update demographic information, leave a message for your provider and more. To use this feature, contact the medical office to get a log in and password. You will need to have an existing e-mail account. After you are “web-enabled” you can access your account online. If you already have online access, click here and you will be redirected to the patient portal.

Need medical advice after office hours?
If you need to contact a provider for after hour care, call our 24-hour on call service by dialing (707) 894-4229 and you will be connected with one of our medical providers.  This service is available in English and in Spanish.

 

 

When is the health center open?
  • AVH Medical Office – 6 Tarman Drive, Cloverdale, CA:
    • The medical office is open Monday through Friday, 8:00AM – 7:00PM.
    • The office is closed for trainings Wednesday’s from 1:00PM – 2:00PM.
    • Lab Services are available for AVH patients only on a drop in basis Monday, Tuesday, Thursday and Friday, 8:00AM – 1:00PM and 2:00PM – 4:30PM (last patient at 4:15PM); Wednesday’s 8:00AM – 12:00PM and 2:00PM – 4:30PM (last patient at 4:15PM).
    • Our Patient Financial Services Department is open Monday, Wednesday and Friday from 8:00-5:00PM and Tuesday and Thursday’s from 9:00AM – 6:00PM.
  • AVH Dental & Administration Office – 100 West Third Street, Cloverdale, CA:
    • Our dental office is open at our 100 Third Street location Monday through Friday, 8:00AM – 5:00PM closed 1:00PM – 2:00PM.
    • Our administration office is open Monday through Friday, 8:00AM – 5:00PM.

Needed Documents for Patient Financial Services

What health services are available?
  • Comprehensive pediatric and adult primary healthcare that includes routine checkups, immunizations, chronic disease management, gynecology (including breast and cervical screening), family planning services for women and men, prenatal and postnatal care.
  • Dentistry – Pediatric and adult preventative and restorative dental care.
  • Behavioral Health services provided by Psychologists and Licensed Clinical Social Workers.
  • Specialty care is available at our medical office through provider off-site referrals for Podiatry, Orthopedics, Neurology, Urology, Physical Therapy, Gastroenterology, Chiropractic, Psychiatry and Endocrinology.
  • Lab services ordered by our providers can be done on-site at the medical office.
Do you offer services for work injuries?
We are available to accept new Worker’s Compensation claims after receiving direction from the employer.  Please notify your employer of any injuries you received on the job and they will refer you to a contracted facility.

 

 

Do you offer services for specialty physicals?
  • We offer comprehensive Sports, DMV and Pre-Employment Physicals.
  • For any other type of specialty physicals, please call our office at (707) 894-4229 for information.
Do you offer confidential services for teens?
Yes, in accordance with California law, a young person, 12 yrs and older, can request and receive confidential contraceptive and behavioral health services and is protected from this information being shared with their parents.  We also provide confidential teen services at our Teen Clinic, located at the United Church of Cloverdale at 439 N Cloverdale Blvd., every Wednesday from 2:04PM to 4:00PM (closed during winter, spring and summer breaks).  The Teen Clinic only serves young people between the ages of 12 and 24.

  • Alexander Valley Healthcare accepts Medicare, Medi-Cal and most private insurances.  If you don’t have insurance, you can also pay out of pocket for your visit.  We provide Sliding Scale fee discounts to income eligible patients.  Call the office to schedule a screening appointment or for more information at (707) 894-4229.  Payment for services is expected at time of visit.  In the medical office, we offer a 30% discount for paying at the time of service!
  • You may be eligible for other programs to cover costs for services.  (Family Pact, CDP: Cancer Detection Program, CHDP: Child Health & Disability Prevention Program).  If you make less than 200% of the Federal Poverty Level and are not eligible for any government health insurance program, you may qualify for a discount visit fee.  Please call (707) 894-4229 to schedule an appointment with our Certified Enrollment Counselor.  (to learn more about Federal Poverty Level)
What is the cost of a dental visit?
  • Alexander Valley Healthcare Dental currently accepts Medi-Cal.  If you don’t have Medi-Cal, you can also pay out of pocket for your visit.  We provide Sliding Scale fee discounts to income eligible patients.  Call the office to schedule a screening appointment or for more information at (707) 894-4229.  Payment for services is expected at time of visit.  In the dental office, we offer a 15% discount for paying at the time of service!
  • You may be eligible for other programs to cover costs for services.  If you make less than 200% of the Federal Poverty Level and are not eligible for any government health insurance program, you may qualify for a discount visit fee.  Please call (707) 894-4229 to schedule an appointment with our Certified Enrollment Counselor.  A Sliding Scale of charges is available and we are able to give you an estimate of the cost for your visit. (to learn more about Federal Poverty Level)
What if I don’t speak English?
Most of our staff is bilingual in English and Spanish.  If you speak another language, telephone interpretation may be available in other languages.
Will I always see the same doctor?
For consistency in your healthcare, we encourage patients to select a Primary Care Provider (PCP) to oversee your care.  If your need is urgent, your provider may not be available, but you will be seen by another qualified clinician.
