Recognition as a Patient-Centered Medical Home

The National Committee for Quality Assurance (NCQA) recognized Alexander Valley Healthcare as a Patient-Centered Medical Home (PCMH). As the name implies, a PCHM is a model of care the places patients at the forefront of care. As part of the 9-12 month evaluation by NCQA, AVH earned recognition by showing a commitment to providing quality improvement within their practice and a patient-centered approach to care. Research by the NCQA indicates that a PCMH improves quality, the patient experience and staff satisfaction, while reducing health care costs.

2019 Million Hearts Hypertension Control Challenge Champion

Alexander Valley Healthcare was one of 17 nationwide honorees of the 2019 Million Hearts® Hypertension Control Challenge by the Centers for Disease Control and Prevention (CDC). The CDC recognized the professional health care providers at AVH for controlling blood pressure in more than 80% of its patients, stating in the award: “These remarkable efforts are critical to preventing and managing heart disease and stroke, two leading causes of death for Americans.” 

Saving lives through better blood pressure control has been a longstanding priority for the CDC and the Million Hearts® initiative. By recognizing the performance of the 17 honorees and sharing their lessons learned, the CDC aims to help other health care professionals achieve the same success in communities nationwide.

2019 HRSA Quality Awards

The Alexander Valley Healthcare (AVH) was recognized with a 2019 Quality Improvement Award by the U.S. Health Resources & Services Administration (HRSA). AVH received a Bronze Award representing the “best overall clinical performance” among the 1,273 health centers in the U.S. 

“We’re very proud to receive the award which is a reflection of the quality, efficiency and value of the health care services we provide to our community,” said Deborah Howell, CEO of Alexander Valley Healthcare. “Our medical, behavioral health, and dental providers are constantly striving to improve patient care. In addition, our administrative staff works tirelessly to offer the highest quality customer service.” 

HRSA presents the Quality Improvement Awards to recognize the highest performing health centers nationwide, as well as those health centers that have made significant quality improvement gains from the previous year. As part of the award, AVH received a grant of $60,994 from HRSA.
