Alexander Valley Health & Wellness Center Reaches Permit Approval Milestone

Excitement is building! Following the submission of our permit applications in December 2025 and nine months of rigorous review, Alexander Valley Healthcare (AVH) is thrilled to announce that we have officially met all requirements for permit approval for the new Alexander Valley Health & Wellness Center.
This month, security fencing is being installed around AVH’s portion of Thyme Square and the adjacent City-owned parcel that will serve as a staging area. PG&E is also working to install temporary electrical power to support the next phases of site preparation.
The first work on site will focus on comprehensive soil stabilization to ensure a strong, resilient foundation for our future facility. In parallel, AVH is working closely with the City of Cloverdale and Cloverdale Unified School District to complete the extension of Washington Street to Healdsburg Avenue, creating safe, dedicated public access to Washington School.
While there is still a long road ahead, seeing physical progress on the property marks an important milestone. Everyone at Alexander Valley Healthcare is deeply grateful to the entire community for its continued support, advocacy, and patience. We look forward to sharing more updates as we bring the Alexander Valley Health & Wellness Center to life!

Have you had your Annual Medicare Wellness Visit?
Medicare Wellness Visits are an important yearly benefit that help older adults stay healthy, independent, and connected to the care they need. This visit is not a full physical exam; instead, it is a personalized prevention visit focused on your overall health, risks, and goals. During a Medicare Wellness Visit, your care team may review your medical history, medications, safety at home, memory concerns, mood, mobility, and recommended screenings or vaccines. Together, you can create or update a prevention plan that supports your health throughout the year. If you have Medicare, scheduling your Annual Wellness Visit is a simple way to take a

Welcome New Provider

Katherine Hylan is a Family Nurse Practitioner and Certified Nurse-Midwife with more than 10 years of experience practicing in California. A graduate of Vanderbilt University School of Nursing, she has cared for patients in both primary care and women’s health inside and out of the hospital, in rural communities and cities, and with diverse populations, languages, and cultures. She also practiced on the frontlines in Boston during the COVID-19 pandemic.
Katherine believes great primary care starts with seeing the whole person. She thinks of each patient as a unique puzzle, bringing together their history, symptoms, imaging, specialist recommendations, and lived experience to understand the bigger picture. She works with patients to identify what matters most to them and develop practical, realistic plans through shared decision-making. Her goal is to help patients navigate a complicated healthcare system while keeping their individual needs and goals at the center.
Her commitment to thoughtful, culturally responsive care was shaped by her experience in the Peace Corps in Senegal, West Africa, where she experienced firsthand what it feels like to navigate healthcare without fully understanding the language. She brings that perspective to every patient interaction, taking time to listen, explain, and make sure patients feel informed and heard.
Katherine is a mother of two elementary-school-aged children and enjoys coaching them on recreational sports teams and participating in community events in her home town of Ukiah. A former collegiate track and cross-country athlete at Bucknell University, she still enjoys an active lifestyle with her family through camping and road trips. She is also an animal lover with a big dog and two cats and enjoys discovering new foods and experiencing other cultures.
Welcome to AVH Katherine!
