Merlo Foundation provides a Matching Grant to the AVH Health & Wellness Center Campaign
Alexander Valley Healthcare (AVH) is thrilled to announce that we have received a generous $50,000 Capital Campaign grant from the Harry A. Merlo Foundation.
Harry A. Merlo’s story is deeply rooted in Cloverdale, CA. Born into poverty in a High Sierra lumber camp, he was the son of Italian immigrants. In 1949, he began his career as a shipping clerk with the Rounds and Kilpatrick Lumber Company in Cloverdale, earning $350 a month. This humble beginning marked the start of a career that would revolutionize the forest products industry.
Harry was deeply committed to the dignity and rights of all individuals. His experiences with discrimination fueled his lifelong dedication to ensuring everyone could live and work in clean, safe environments, enjoy the fruits of their labor, and voice their opinions freely. This respect for human dignity became a hallmark of his career and life, and it remains a cornerstone of the Merlo Foundation.
Though Harry spent his final years in Oregon, his family’s connection to Sonoma County remains strong. The $50,000 from the Merlo Foundation is a matching grant, intended to inspire additional support for the Alexander Valley Health and Wellness Center. We are profoundly grateful for this opportunity and the legacy of Harry A. Merlo that continues to inspire and uplift our community.

AVH is grateful to Redwood Credit Union for their grant funding to support our new Training/Meeting room! AVH staff and providers undergo extensive and ongoing training to stay up-to-date on the ever-changing world of healthcare and technology. Now we can hold group training sessions and meetings in a spacious area with new furnishings. Thanks RCU for partnering with us to create a better p!ace to work and learn!
Meet our Providers
Monica Mendiola recently joined AVH as a Physician’s Assistant (PA) providing virtual care to our patients. Monica trained at UC Davis. She has more than three years of clinic experience and speaks fluent Spanish. Monica will see ER/Hospital follow-ups, same day patients, review abnormal labs and imaging results with patients and take care of those in need of provider’s visits before medication refills.

Program Updates
On July 22, the California Department of Health Care Services awarded $37 million to 49 clinics providing Enhanced Care Management (ECM) and Community Supports services to Medi-Cal members.
These Awards were made as part of the Providing Access and Transforming Health (PATH) Capacity and Infrastructure, Transition, Expansion, and Development (CITED) initiative’s work to help build, maintain, and scale clinics’ capacity to participate in the Medi-Cal delivery system.
Alexander Valley Healthcare is a proud recipient of funding from this grant to build support for the delivery of Enhanced Care Management to our highest need Medi-Cal managed care members through whole-person care coordination teams. Read more here.
