March is Mental Health Month, a time to recognize that emotional wellbeing is essential to
overall health. At Alexander Valley Healthcare, we care about the whole person—not just their
medical symptoms.
The AVH Behavioral Health team includes a Licensed Clinical Social Worker (LCSW), a staff
psychologist, a psychiatric mental health nurse practitioner, a substance use disorder (SUD)
counselor, and an integrated behavioral health case manager. Working side by side with our
medical and dental providers, they help ensure patients receive coordinated, compassionate
care that addresses both physical and mental health needs.
To better support patients with complex needs, we have launched the CalAIM Enhanced Care
Management program. This initiative helps us go beyond traditional medical care by connecting
patients with resources, care coordination, and ongoing support tailored to their individual
situations.
One recent patient shared, “The AVH Behavioral Health team helped guide and advise me
when my mom could no longer live safely alone. They listened, directed me to community
resources, and stayed in touch with followup visits and phone calls. Because of their support,
my mom is now in a safer setting, and I feel less overwhelmed knowing we are not alone.”
In preparation for the new Health & Wellness Center, (AVH) has received state capital funding
through the Department of Health Care Services’ Behavioral Health Continuum Infrastructure
Program (BHCIP). This investment will help us expand mental health services in our new facility,
increasing access to care for Cloverdale and surrounding communities.
